Referrals & Consultations
The VMTH welcomes companion animal, equine, and food animal referral cases from private veterinarians. Either the veterinarian or the client may call to schedule an appointment. Veterinarians should provide a referral form summarizing the animal’s clinical signs and the diagnostic procedures and/or treatments that have been done prior to referral.
Veterinarians will be contacted both by telephone (within 24 hours) and by letter (within one week of the animal’s discharge). Faculty are available to consult by telephone with veterinarians regarding the care of animals that have not been evaluated at the Veterinary Medical Teaching Hospital.

To Our Referring Veterinarians:
In order to serve you and your clients better, we are pleased to announce our new feedback survey for referring veterinarians. This survey allows you to provide input specific for each referral experience. Thus it is an ongoing tool that you may utilize as many times as you would like. There are both pre-set questions as well as space for your own entries. You may fill out as much or little as your time allows. Responses will be delivered to our referring veterinarian coordinator, Ms. Connie Sievert, and distributed according to the service to which you referred your client and patient. If you would prefer to speak directly to us, please call and we would be more than happy to discuss your experiences.

If you would prefer to speak directly to us, do not have computer access, or would prefer to receive a paper copy of the survey, please call Ms. Connie Sievert at 1-866-895-7267.
Thank you again for your tremendous support. We are honored to be a part of your veterinary healthcare team!
